To qualify for the grant, your business must:

• Be physically located within the Downtown Plainfield SID boundaries
   
• Have experienced storm-related damage in July 2025
   
• Be currently operating or have a plan to reopen soon

Be prepared to upload the following documents:

1. Completed Online Application
    
   • Business name, address, and contact info
      
   • Description of your business and years in operation
      
   • Narrative explaining the storm damage and how funds will be used
      

1. Proof of Storm Damage
    
   • Photos or videos of the damage
      
   • Repair estimates or invoices
      
   • Optional: Police reports or insurance claims
      

1. Receipts for Emergency Repairs (if applicable)
    
2. W-9 Form for payment processing

Once approved:

• You’ll sign a short grant agreement
   
• Funds will be disbursed shortly after
   
• Within 60 days, you’ll submit a brief Use of Funds Report, including:
   
  • Receipts or proof of repairs
     
  • Photos of improvements
     
  • A short summary of how the grant supported your business
